INTRODUCTION
The contextual information from the case revolving around Emily who is a 35 years old female healthcare administrator; she is working in a large urban hospital within Wales. Emily is a single parent and further she has 2 children; during second birth Emily suffered from post-natal depression which led to six months off from work. Emily performs her duties while maintaining autonomy, dignity and honesty; she is accountable for overseeing varied department that involves billing, patient admission and medical records. Emily is passionate about encouraging person-centred care approaches and assures that care is tailored as per individual’s needs. Despite performing effective practices in own job role Emily has been suffering from workforce shortages and this led to increase work pressure, ineffective quality care and staff burnout at the setting.
From recent audit at hospital; it was evident that 50% of the patient felt dissatisfied from the services of care. As a result, Emilys started to feel pressurised and further she started to rely on alcohol and illegal substances. Moreover, Emily has isolated herself from her family. The present case evaluation will focus on Emily’s situation by considering sociology, psychology, anthropology aspects. Furthermore, emphasis would be made on medical and social models while considering social factors leading to Emily’s behaviour.
BACKGROUND
Emily struggled in her job role; looking upon children and maintain job accountabilities within setting created pressure on her. Despite performing fair and consistent practices Emily has not received any promotion at workplace. Whilst, during COVID-19 she focused on implementing telehealth services which supported patients living in remote areas. Emily never received an appreciation and appraisal for her work; although, she continued to perform her practices. However, due to resources shortage she faced criticism; at this point she is highly depressed.; Emily always ensured that patients are receiving quality care; being in the job role of administrator she has performed the accountabilities of manager. From the case context it is evident that throughout the past Emily has suffered due to lack of support from her partners.
Further, she was never recognised for her performance; this might be due to cultural dimension as she belongs from rural areas or due to her gender (Thorpe, 2018). At present, behaviour of Emily impacting her decision and she is lacking in resilience capabilities. Case information has been significantly highlighting about the gender-based difference in which women are not treated equally when they want promotion. The conditions in which Emily was working proves to be inappropriate. She was stuck between caring child and managing workload at setting. The concepts related to case has been significantly highlighting about the Gender-based difference within health and social care. It is a topmost concern within healthcare setting that requires significant emphasis.
SOCIOLOGY
Sociology determines about the aspects of human social life that signifies about the ways in which people interact with each other. As per Functionalism theory of Emile Durkheim society is considered as the system in which different parts are inter-related with each other and works combinedly for maintaining stability order within society (Nolan, 2011). By considering this theory it can be said that Emily’s behaviour of drinking and consuming illegal substances was the result of lack of appreciation in her work role. She faced criticism when patient was dissatisfied however, during COVID-19 and afterwards she has promoted person-centred approaches. Whilst, she never received promotion or appreciation. Hence, all of these situations led to develop drastic situation for her and she prefer isolation. Individual’s behaviour creates subsequent impacts on their action and similar aspects has been witnessed within case of Emily (Weeks, 2014). The cultural dimension upholding the notion of disparities due to different background; she was from rural areas and despite she achieved varied milestone by promoting person-centred care. This clearly depicting that belonging from deprived areas can act as a barrier in receiving managerial position.
As per Conflict theory of Karl Marc it is said that society is considered as a constant state of conflict because of limited resources among social class (Whitehead and Dahlgren, 2021). The limited resources at the setting lead to increase work pressure on Emily and existing staff; thus, here limited resources are the cause of concern behind raising. The working environment developed for a woman seems different from men; if there was a male administrator in the role of Emily then; the things could have gone different (Yao et al, 2022). The higher administration team might have supported male administrator as they like to see men at leadership position. The positivism approach of society has developed a structured thinking in mind of individuals that women are not able to hold professional responsibilities due to household responsibilities (Yao et al, 2022). In Emily’s case she had to look upon her children and further she was on off for 6 months after delivering second baby. All of these circumstances develop a perception in the mind of upper administrators that she would not be able to perform duties in appropriate manner with all these responsibilities. Considering the presented information, it can be said that structured pattern of society and lack of resources within community creates gender-based differences within health and social care settings and same aspects witnessed in Emily’s case.
PSYCHOLOGY
Psychology signifies about the interaction among mind, behaviour and human experience. Emily’s issues can be understood in depth while considering psychological theoretical instances. BF Skinner’s theory articulates that an individual is exposed to stimulus which leads to evoking response and then this response is reinforced and takes the form of human behaviour (Ndugga and Artiga, 2021). Different experiences in Emily life led to develop present situation. Emily was feeling pressurised and due to lack of support she started to rely on alcohol. Looking at the upbringing of Emily; it can be said that; she comes from a traditional family and she did not want to develop a situation of shame for her family and thereby, she isolated herself. Emily was a single parent and at this moment there is no one who can look upon her.
The humanist theory of psychology provided my Maslow depicts that- individuals wants to reach their complete potential which concerned with five hierarchical needs. As per Maslow hierarchy needs; human behaviour impacts their actions; Maslow has emphasised on five topmost needs and those are- physiological, safety, social, esteem and self-actualization (Williams et al, 2022). In Emily’ case she was able to fulfill her psychological needs. However, in terms of safety needs it was identified that she is lacking in job security as Emily was struggling receiving promotion. Social needs signify about the extent of care, affection and support; these needs of Emily have not been ful-filled as she was a single mother.
Esteem needs depict about sense of worth that is gained from recognition of achievements. Once esteem-needs are met then, this led to full-fill self-actualization needs. In case of Emily, it has been witnessed that her esteem needs were not full-filled as she never received appreciation and recognition (Mercer et al, 2021). As a result, she was not able to full-fill her self-actualisation needs. Emily’s safety, social, esteem and self-actualisation needs were not met and this lead to impact her behavioural pattern. In past she was so concerned about patients however, at preset she is pressurised due to dissatisfaction of patients. Lack of esteem in her led to develop trauma and she choose to drink alcohol for escaping from this situation. In case of Emily, esteem needs are primarily responsible for her current situation (Mercer et al, 2021). At the setting she consistently shown her efforts but in return she never received any gratitude. She was never able to feel proud on herself and this clearly signifying about gap existing in health and social care setting.
Gender differences within health and social care leads to unfair treatment of workers due to different gender. Females primarily suffers from discrimination and this results in reducing quality of healthcare services (Marmot,2022). Emily suffered from gender-based discrimination and at last due to lack of resources the quality of services got reduced. The behaviour and actions of Emily was the result of varied circumstances she faced in past and present.
ANTHROPOLOGY
Feminist Anthropology explicated as the subfield of anthropology that emphases on feminist perspective; it critiques male-centred approaches by highlighting wider contribution of women within society (Ocloo et al, 2021). This theory aims of reveal challenges faced by women while focusing on the perception of women and the ways in which social structure shapes them. The key domains related to feminist anthropology are gender bias, intersectionality and power dynamics. Implication of this theory in Emily’ case can be done for understanding the gender-based issues within health and social care. However, in return she received nothing which developed a sense of hopelessness in her. Marvin Harris proposed cultural materialism approach; it is a theoretical approach which states that; materialistic conditions of society is considered as primary factors for shaping social structures and cultural beliefs. The three level of culture has been identified with theory and those are-infrastructure, structure and superstructure (Hsieh and Shuster, 2021). Infrastructure concerned with demographics, economy and technology. Whereas, structure associate with social structure and organisations. Superstructure concerned with ideological aspects such as art, beliefs and religion. In Emily’s case “structure” is witnessed as a major factor that led to develop such situation. Lack of resources in the setting influenced the practices at the setting (Hsieh and Shuster, 2021). Due to staff shortage Emily was not able to meet the needs of patients which led to huge dissatisfaction.
By considering the structure component this can be said that; there was lack of resources within setting which led to develop such situation. The loophole was within system whilst, Emily has to suffer from this; from analysing anthropology instances; it can be said that; this supports in highlighting the role of women healthcare leaders, nurses, professionals and practitioners within health and social care. Further, it determines about the gaps existing within system so that appropriate decision can be taken.
MEDICAL MODEL AND SOCIAL MODEL
The Medical model depicts that people are disabled due to their impairments and differences. It focuses on the individual impairment rather than group differences. On the other hand, social model views disability as a result of societal behaviour and issues which lead to impacts individuals (Saeed and Masters, 2021). This emphasis on the differences existing within society. Medical model aims to fix individual issues while social model’s motive is to remove societal barriers. In Emily’s case social model can be considered as her situation was the result of loopholes existing within system. Gender-based differences are considered as the part of societal pattern which keeps passing on (Ocloo et al, 2021). Despite implementing different policies, it is still persisting and females’ workers are suffering from this. Thus, this significantly depicts that it is important to ensure focus on this area and accordingly, appropriate actions are required to be taken.
SOCIAL FACTORS
Social factors can be described as social determinants of health; this are considered as non-medical factors that influences health and wellbeing of individuals. It involves education, lack of access, unemployment, working life conditions, housing, inequality and income. Emily was depressed after knowing the patients are dissatisfied with the services (Saeed and Masters, 2021). However, she performed her responsibilities appropriately but due to ineffective working conditions she faced such issues that led to develop depression for her. There was drastic influence on Emily’s health and wellbeing was witnessed. Furthermore, inequality found to be another social factor in Emily’s case which led to impact her wellbeing. Gender-based inequality has been faced by Emily; she had two children. Throughout starting Emily performed responsibilities by her own whether it was about looking upon children or scheduling task at workplace (McCartney et al, 2021). From the Emily’s case it has been identified that social factors play pivotal role in impacting individual’s wellbeing. Hence, there is need to emphasis on social factors so that appropriate actions can be taken and significant support to individuals like Emily could be provided.
FINDINGS AND DISCUSSION
From the present case evaluation, it has been identified that; Emily faced varied issues at the setting and that significantly depicting about Gender-based discrimination. Different perceptions have been drawn from sociological, psychological and anthropology theoretical instances. The existing pattern within society underpinning varied differences between men and women; this has been witnessed in current case context (Homan, Brown and King, 2021). Emily have not recognised for her performance and due to lack of resources she faced consecutive challenges and at last patient were dissatisfied with the services. This led to develop trauma for her and she was suffering with anxiety and depression and therefore, she relied on alcohol for escaping from this situation. The sociological theories revealing that different context of society are interlinked with each other and collectively this lead to influence an individual’s behaviour and actions. Similarly, in case of Emily, her actions and behaviour were the result of lack of support, recognition and affection (Green, 2023). Her needs were not ful-filled specifically esteem needs and this resulted in reducing her confidence.
Feminist Anthropology revealed that contribution of Emily was ignored by healthcare setting because she was a woman. Society focuses on male-centred approaches and hides female contributions. Thus, this is the significant challenge in present era that should be focused out within health and social care setting (Green, 2023). The case of Emily underpinning major loopholes existing within society; it is depicting that lack of resources, ineffective working condition and gender-based biasness can develop drastic impact on an individual and this further reduces quality of healthcare services. Health and social care practitioner can address these issues by reporting them to concerned individual (Green, 2023).
CONCLUSION
Conclusively; this can be said that present case study has developed significant insights related to challenges persisting in health and social acre. The issues of Emily are significantly depicting about struggles she had for becoming a manager. Despite performing effective practices and empowering patient she was never appreciated and recognised. Ineffective working condition and lack of resources within system led to impact her practices due to which patient was dissatisfied. This case has drawn the attention on a sensitive topic “gender-biasness” females within health and social care settings are still facing complexities in gaining leadership position and this is articulating about the loopholes existing in society. A robust strategy must be implemented within settings and on the basis of individuals potential they should be promoted. Furthermore, resource allocation is equally important as it influences quality of services.
